4/3/2023 EnchantaVaughn Approved Behavior: Upon QMHP arrival, client used her free time to rest and relax before coming to her session. Client presented himself to be in a good mood, as evidenced by her optimistic attitude and excitement to see her counselor. Client was given a compliment by her mother who noticed her change of behaviors. Client expresses positive statements while contributing to the therapeutic discussion. Client seem to be doing a lot better focusing on herself and ignoring distractions in the home. Behavior: Client displayed good listening skills while counselor reviewed her objectives. Client was supported with displaying a good attitude in the home, as well as, changing her approach to mental health tasks. Client has shown interest in sharing her ideas and taking responsibility for everything she has done to make herself a better person. Client seems to be handling herself well in the home and following the rules and adapting to the in-home schedule and routine. Intervention: QMHP began the session by inquiring about clientâs current thoughts, feelings, and emotions. QMHP inquired about clientâs day and what things sheâs doing to make it a good day. QMHP educated client about what she can do to make being at home fun and interesting, such as reading her favorite book, drawing in a journal, coloring pictures, or practicing the mindful behaviors learned in previous sessions. QMHP provided and demonstrated positive and empathetic acceptance, as talk therapy and active listening skills were applied. Intervention: QMHP facilitated family session to encourage open communication with client and mom to process on various ways to express feelings and thoughts in a controlled manner. QMHP discussed with client and mother on assertiveness and respectful behaviors towards each other. QMHP reiterated with client about completing assigned chores in the home daily so that she doesnât get in trouble and can enjoy more time outside the home as a direct incentive to her contributions. Response: Client expresses good statements about her day saying she was doing her work, using her manners, and being fair to others. Client said she was feeling good and plans to utilize her time to get as much done early so that she can have the rest of her free time to herself. Client displayed management of individualized behaviors as she interacted appropriately within the session. Client have been doing a better job controlling impulsive and hyperactive behaviors, due to utilizing relaxation techniques and staying on task. Response: The client has not overreacted with anger to minor frustrations or irritants which helps her family feel closer to her and able to be by her side and support her. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided and showing gratitude for her familyâs support. Client was attentive as her counselor assisted with reviewing her in home schedule and preparing for the next intervention. As the client developed understanding of expectations, she reported increased satisfaction and function.
